Our re-hospitalization rate is 12.8%. This is lower than both the state (16%) and national rates (16.5%).
We prefer that you fax referral forms to us.
Because we are not a 24-hour business, you can expect an answer:
- During Business Hours – 15 Minutes Max
- After-hours – 8:30 AM the following morning
- Weekend – The following Monday
We accept referrals based on:
- Medical Necessity
- Home Bound Status, and
- MD Orders
To accept a referral we require the following documents:
- MD order for Home Health
- History & Physical/Progress Notes from MD or Hospital
Within 24-hours of accepting the referral
How is your agency going to make sure that the patient I refer doesn’t wind up back in the hospital again?
Depending on the needs of the patient, we will see the patient more frequently (front-load visits). We will also make sure that the patient has our phone number to call us so that they don’t have to call 911 and potentially be readmitted to the hospital
After a patient is seen, our clinical team discuss what the patient’s needs are and coordinates that care with the patient-PCP.
