Does the agency provide overnight and/or 24-hour care?
What is your re-hospitalization rate?
Our re-hospitalization rate is 12.8%. This is lower than both the state (16%) and national rates (16.5%).
Do I have to fax you the referral forms or will someone come pick them up?
We prefer that you fax referral forms to us.
If I call you with a referral how soon will I get an answer if you accept?
Because we are not a 24-hour business, you can expect an answer:
- During Business Hours – 15 Minutes Max
- After-hours – 8:30 AM the following morning
- Weekend – The following Monday
How do you determine which referrals you will take?
We accept referrals based on:
- Medical Necessity
- Home Bound Status, and
- MD Orders
Do you pick up referrals over the weekend and holidays?
What paperwork do you require to accept a referral?
To accept a referral we require the following documents:
- MD order for Home Health
- History & Physical/Progress Notes from MD or Hospital
How soon will you see a new patient referral?
Within 24-hours of accepting the referral
How is your agency going to make sure that the patient I refer doesn’t wind up back in the hospital again?
Depending on the needs of the patient, we will see the patient more frequently (front-load visits). We will also make sure that the patient has our phone number to call us so that they don’t have to call 911 and potentially be readmitted to the hospital
Are your community liaisons credentialed?
What are your protocols to control pain, infections, re-hospitalization?
After a patient is seen, our clinical team discuss what the patient’s needs are and coordinates that care with the patient-PCP.
Where do you provide services? Pahrump? Mount Charleston? Laughlin?
