Home health care must be ordered by a Nevada Board Certified MD (Doctor of Medicine) or DO (Doctor of Osteopathic Medicine). Referrals can be made via a doctor’s office, hospital, clinic, or anywhere else an MD or DO practices.
In order for a patient to sign on, Sonoma Home Health requires 3 things:
- The MD/DO order for home health
- Patient information (such as name, address, and phone number)
- Medical history and physical
Each patient receives intermittent skilled care; the frequency is determined based on a care plan that’s determined after an evaluation has been completed. During each visit, the patient will be visited by clinicians (skilled nurses) for 45 minutes – 1 hour.
Personal care such as bathing, dressing, and food preparation. Home health can provide home health aides on a limited basis depending on diagnosis and need, but it isn’t long-term and must be in conjunction with a need for skilled nursing based on a doctor’s referral.
Medication. Prescriptions are not covered by home health.

In the state of Nevada, home health care providers must complete 75 hours of Medicare-certified training.
